1. Are you looking to adopt an inside only cat or an inside/outside cat?
2. Do you rent or own your home?
3. Are you aware and understand that changing an animal’s environment may cause the animal to have accidents, especially in the first few days? You must answer Yes to continue.
4. Have you ever owned a pet before?
5. If you currently have any animals, are they up to date on vaccines?
6. Have any of your pets been hit by a vehicle before?
8. Have you ever rehomed any of your animals for any reason?
11. If there were any reasons that you may be unable to keep the animal, do you agree to contact Pet Projects immediately? You must answer Yes to continue.
12. Do you accept that Pet Projects makes no guarantees in reference to health and/or temperament of the animal? You must answer Yes to continue.
Agreements

Adopters must agree that all animals are adopted as is and must assume all responsibility for treatments of all preexisting conditions as well as any medical, physical, or temperamental conditions that may arise once adopted. Adopters must also agree that Pet Projects cannot be held liable in regards to the analysis of this form and/or placement.

While Pet Projects makes every effort to insure that only healthy and well-adjusted animals are placed into homes, we cannot guarantee the health or temperament of any animal and cannot be held responsible for temperament changes that may occur or any medical expenses that may arise.

By submitting this application you give Pet Projects permission to check the information provided as well as permission to contact any veterinarians and/or organizations and/or references given on this application as it relates to ownership of an animal.
